Wedding: Facility/Room Request Form

(Whole Church, Multi-Purpose Room (Gym), Sanctuary, Kitchen, etc.) 

McCoy Memorial Baptist Church (MMBC) facilities are open for use to MMBC members and regular attendees. MMBC is not open for use by outside organizations. There will be a $250 security deposit and possibly a custodial fee. Those using the facilities must adhere to the following rules and restrictions. 
1) All activities should be God honoring and appropriate and must be in compliance with MMBC’s Statement of Faith. No alcohol, tobacco or non-prescription drugs allowed on property. No smoking is allowed anywhere on church property.
2) Those wanting to reserve the facilities must fill out the "Facilities Use Agreement". You must first meet with a MMBC Pastor and MMBC’s Wedding Coordinator to discuss the needs and the resources available at our facility such as the number of chairs and tables required, the allowable occupancy, applicable church use policies, etc. You may also be asked to meet with MMBC’s Kitchen Coordinator for any large events requiring the use of the kitchen. 
3) An open dance floor will not be permitted. The following dances will be permitted: a dance between the bride/groom, father/bride, mother/groom. 
4) A responsible adult (21 or older) is required to be present during the entire event and monitor all activities. 
5) Dates are reserved on a first come, first served basis. A "Facilities Use Agreement" must be filled out. If the requested date is available, your request will be tentatively placed on the church calendar and the request form presented to the Trustee Board for final approval. 
6) It is your responsibility to arrange entrance and exit of the building with the responsible adult in charge of the activity / (See point 5 above). It is the responsibility of this adult to obtain the needed key(s) and to see that the building is unlocked before and locked up after your event. To minimize the possibility of an unwanted intrusion we strongly recommend the building entrances be locked during the event. 
7) You are responsible for all set up and tear down in all areas which you use. Please make sure the rooms which you use are returned to their former set up when you are finished. This includes but is not limited to removing trash and sweeping floors. A detailed check sheet is posted in the kitchen. 
8) You are responsible to supply all plates, cups, coffee and other supplies that you may need for your event. Please do not use church supplies for any non-church related events. 
9) Any damage to the facility, furniture, or equipment must be reported via a Damage Report Form located in the kitchen. This must be submitted to the church office within 24 hours. It may be placed in the church secretary's tray in the cabinet in the church foyer. If damage is a result of carelessness or improper use of facilities you may be charged for the repair or replacement of items affected. 
10) If personal injury occurs, take immediate action to deal with those injured. Do not hesitate to call 911 if there is any doubt as to the seriousness of the situation. Emergency First Aid Kits are located in the hallway near the kitchen, in the lower hall near the children's wing and in the upstairs foyer area of the church. An Accident Report (located with the First Aid Kits) must be completed as soon as the injured are cared for. It must include the name(s) of the injured parties and signatures of at least one and preferably two adults who witnessed the event. It must include a detailed description of what happened and the actions taken to provide aid to the injured. 
11) In order to guard our Pastors’ busy schedules and private family times, we ask that you do not call on them to assist in locating supplies, helping with setup or tear down, unlocking, locking, turning off the lights, etc. If you anticipate the need for such help please make that need known before the event so appropriate assistance can be made available. There may be a charge for this if it involves extra duty for church employees. 

McCoy Memorial Baptist Church Multi-purpose Room/Kitchen Rules 
1) A Facilities Use Agreement must be completed and submitted to the Church Secretary to schedule your event before using the multipurpose room or kitchen. 
2) There will be a “designated person” at every event in our multi-purpose room. This person is fully responsible to make sure that all the following rules are completely carried out. All participants in an activity are only to be in the gym or bathroom areas and not in any other part of the church. 
3) At the end of every event all gym and kitchen lights must be turned off, all doors locked.
4) If for some reason any other equipment is used at the end of the event, all basketball and volleyball equipment anything else must be put back neatly in its designated place in the storage room. Equipment should be used only for what it is designed for – volleyballs should not be used for kick balls, etc. 
5) Only non-marking gym shoes are allowed when playing games on the gym floor. Stiletto heels are strictly prohibited on the gym floor as they can damage the floor. 
6) At the end of each event, the gym floor must be swept clean with the push broom (dust/mop) and all dust and debris picked up and put in the trash can. 
7) When using the gym for an event which requires chairs and tables, the group hosting the event is fully responsible for set up, tear down, and putting all tables and chairs away. Make sure the multipurpose room is left just as it was found. Tables and chairs must not be dragged across the floor. Please carry them to prevent damage to the floor. 
8) The Trustees may require a setup and/or cleaning fee if a church employee is needed to supervise the setup/tear down or cleaning needed for your event. 
9) If using the kitchen, all items used must be washed and put away where they were found. Make sure the kitchen is left just as it was found. 10) Any damage to the facility, furniture, or equipment must be reported via a Damage Report Form located in the kitchen. THE WORD OF GOD

THE FATHER

JESUS CHRIST

HOLY SPIRIT

 CREATION
We believe all creation came into being in seven days through the eternal Son and is presently sustained by Him.

MANKIND
We believe that man was made in the image of God, originally sinless and pure, but sinned in Adam as the natural head of the race and is now sinful by nature and by choice and in need of salvation.

SALVATION
We believe in salvation by grace alone through faith alone in the Lord Jesus Christ alone through the merits of his atoning blood and not on the basis of human merits or works.

SECURITY
We believe that all the redeemed, once saved, are kept by God’s power and are thus secure in Christ forever. Those who once professed faith and subsequently deny the Lord demonstrate by their going out from us that they were never truly saved.

THE CHURCH
We believe spiritual gifts are given by the Holy Spirit to believers when they are saved.  Those gifts are given for the unification and edification of the church, never for its fragmentation.
We believe that the universal church of Christ is a spiritual body of which He is the head and of which all regenerated people are members.
We believe that the local church is a company of believers in Jesus Christ, immersed on a credible profession of faith, and voluntarily associated for worship, prayer, work and fellowship.
We believe that to these local churches were committed for perpetual observation the ordinances of baptism and the Lord’s Supper and that God laid upon these churches the task of proclaiming to a lost world the Gospel of Jesus Christ.

THE ORDINANCES
We believe in two church ordinances:  baptism, being the immersion of the believer in water, in the name of the Father, and of the Son, and of the Holy Spirit, thus symbolizing the believer’s previous experience of regeneration through faith and his union in the death, burial and resurrection of Christ as set forth in Rom. 6:3-11; the Lord’s supper, instituted by Christ, being a symbol of His atoning sacrifice.

LOCAL CHURCH AUTONOMY
We believe that the local church is independent and autonomous, and its Statement of Faith must be free from interference by any ecclesiastical or political authority and that, therefore, church and state must be kept separate as having different functions.

We believe in the resurrection of the body, the eternal existence of all men either in heaven or hell, in divine judgment, in rewards and punishment and the certain fulfillment of all Bible prophecies.

We believe that we “should earnestly contend for the faith once delivered unto the saints,” and that we should preach the Word, being urgent in season, out of season, reproving, rebuking, exhorting with all longsuffering and doctrine.

BIBLICAL MARRIAGE AND SEXUALITY
We believe that God created marriage to be between one man and one woman. God created sexual intimacy to happen solely within the covenant of marriage. All other forms of sexual intimacy are sin.
